  • Outpatient preventive & acute care, management of some chronic & long-term medical conditions, health maintenance, and some prescription assistance.
  • Monday 8:30am-5pm., Tuesday 3:30pm-8pm. Wednesday –12:30pm-5pm, Thursday 8am-1pm. Friday 8:30am-1pm.
  • Appointments can only be scheduled after eligibility screening has been accomplished. Eligibility screening is by appointment only. The following documentation is required at eligibility screening appointment:
