Outpatient preventive & acute care, management of some chronic & long-term medical conditions, health maintenance, and some prescription assistance.
Monday 8:30am-5pm., Tuesday 3:30pm-8pm. Wednesday –12:30pm-5pm, Thursday 8am-1pm. Friday 8:30am-1pm.
Appointments can only be scheduled after eligibility screening has been accomplished. Eligibility screening is by appointment only. The following documentation is required at eligibility screening appointment:
Copy of current month’s bank statements.
Oregon Driver’s License or other proof of Deschutes County Residence.
Pay stubs for the last three months.
VIM provides outpatient primary care, mental health screening and treatment, chronic disease management and some referral services.
Office Visits – no charge, donation requested.
Prescriptions – no charge, donation requested.
